Registration Cancellation - Deceased

If you wish to have someone removed from the voter registration roles because they have died you need to fill out a Registration Cancellation Form. The form contains the following required statement: "I hereby declare, under penalty of perjury, that I am a Registered Voter and according to my personal knowledge of belief... "(include the following information) 

The name under which the person was registered, their address, date of birth and date of death if known.

The cancellation form can be sent to the Secretary of State's Office in Olympia or the Spokane County Elections Office at 1033 W Gardner Ave, Spokane, WA 99260.
